    I found this copy of Sam&Max on Ebay....seems a legit copy but I'm not sure about the CD-Room
    in the jewel case...

    This copy indeed looks complete.

    ...but it looks like the owner cut the paper sleeve to fir into a plastic case... surprisingly common thing to see...
